A 24-year-old male patient received BNT162b2 (COMIRNATY), as dose 1, single (Batch/Lot number: unknown), in right arm and as dose 2, single (Batch/Lot number: unknown), in right arm for covid-19 immunisation. The patient's relevant medical history included: "previous surgery on his left arm" (unspecified if ongoing); "Anxiety" (ongoing). Concomitant medication(s) included: CANNABIS taken for anxiety (ongoing). The following information was reported: POST-ACUTE COVID-19 SYNDROME (disability), outcome "not recovered", described as "feels like he has long covid symptoms"; FATIGUE (non-serious), outcome "not recovered", described as "He has extreme fatigue"; DYSPNOEA (non-serious), outcome "not recovered", described as "it is hard to breathe sometimes"; COUGH (non-serious), outcome "not recovered", described as "He has had a underlying cough that he has had since receiving the vaccine"; MEMORY IMPAIRMENT (non-serious), outcome "not recovered", described as "his memory has gone to crap and can not remember anything that well. Stated he guesses it could be called memory loss or foggy memory."; LIMB DISCOMFORT (non-serious), outcome "unknown", described as "He said that when he received the vaccine it made his arm feel not good.". The patient underwent the following laboratory tests and procedures: Weight: 120-130 lbs. Clinical course: Caller is a consumer and said that he received the Pfizer Vaccine for Covid and it has pretty much destroyed his life. He is looking for compensation because he is disabled. Has been trying to get on with disability for like 3 years and he is major debt now. He does not have a primary care physician. Stated that he doesn't have a running vehicle and said that he is trying to get a physician currently. Age at onset: 24-25. He got his first shot in (withheld) and received the 2nd shot in (withheld). Feels like he has long covid symptoms. He has extreme fatigue and it is hard to breathe sometimes. It takes him 1.5 to 2 hours to pry himself out of bed. He has had underlying cough that he has had since receiving the vaccine. Another bad part of the covid type symptoms, his memory has gone to crap and he cannot remember anything that well. He guesses it could be called memory loss or foggy memory. No matter how much he sleeps they symptoms seem to stay bad or get worse. Stated that he deserves financial compensation. Does not have the vaccine card to provide a NDC, Lot, or expiry date for the 2 doses that he received. Does not remember the dates he received the dose. He received the vaccines between 2019 to 2020 is all he can remember. It was around the time they were all saying to get the covid vaccine. He said that when he received the vaccine it made his arm feel not good. He feels like he got the doses in his right arm because he had previous surgery on his left arm and doesn't like needles in his left arm. After he learned it was a government thing he stopped taking the vaccines after the 2nd dose. He is really struggling and this is not ok. The information on the batch/lot number for BNT162b2 will be requested and submitted if and when received.
