A 16-month old male received a dose of Pentacel (DTAP-IPV/Hib) vaccine on 05/11/2026. Following administration, a potential vaccine preparation error was identified involving the Pentacel not being reconstituted with a required diluent (Hib component) prior to injection. It cannot be confirmed with certainty that this patient's dose was not reconstituted, however, concern was raised due to identification of a similar potential preparation issue involving another patient during the same time frame. As a result, there is a possibility that only one component of the vaccine may have been administered without the diluent. At the time of reporting, no immediate adverse reactions have been observed.
