Pt was to received 4 year old vaccines at the health center today. Mom was instructed to hold the pt/child securely, another nurse came in to assist and helped hold the child. Dtap-IPV was partially administered due to child move abruptly and tried to get away. Mom was instructed to schedule a nurse visit for vaccination in one month.
