The patient's pertinent medical history, concurrent conditions, concomitant medications, previous drugs reactions or allergies were not reported. On 05-May-2026, the patient was vaccinated with Measles, Mumps, and Rubella (Wistar RA 27-3) Virus Vaccine, Live (M-M-R II), lot #Z006283 has been verified to be a valid lot number, expiration date: 19-Mar-2027, administered for prophylaxis (dose, route of administration and anatomical site of injection were not provided). Reconstituted with sterile diluent (BAXTER Sterile Diluent), (indication, expiration date, and lot # were not reported). The vaccine was administered after a temperature excursion in which it was exposed to 65°F for 6 hours (Product storage issue). No previous temperature excursions or additional adverse events were reported (no adverse event).
