Symptom narrative

Client presented to clinic with records to transribe into State Vaccine Database. Transcribing nurse entered two historical varicella vaccines into EHR and only of them made it into State Vaccine Database due to slow internet signal. Vaccinating nurse noted a final dose of Varicella was needed based off the State Vaccine Database printed at the time of screening. Vaccinating nurse contacted FOC to discuss administration error and parent understood and has no further questions. FOC reports no adverse symptoms.
