Symptom narrative

Only Meningococcal B portion of Penmenvy combination vaccine may have been given instead of both Men B and Menveo. An extra dose of Menveo portion was found in the box. There were two patients that received doses out of the box of 10, but we can't say for sure which patient received the full dose.
