The patient's medical history, current condition and concomitant medications were not reported. On 04-Nov-2025, patient received improper storage of rotavirus vaccine, live, oral, pentavalent (ROTATEQ) vial, 2 (unit not reported) orally (lot number: 2142135 with expiration date: 28-May-2026, dose text: 2ml/once); and was vaccinated with improper storage of diphtheria and tetanus toxoids (+) acellular pertussis (+) inactivated poliovirus (+) hib conjugate (mpc) and rhbv vaccine (VAXELIS) injection 0.50 mL (lot number: U8309AA with expiration date: 30-Sep-2027, dose text: 0.5ml/once, route unknown), both for prophylaxis (Product storage issue). No additional adverse event reported. Administered doses of rotavirus vaccine, live, oral, pentavalent (ROTATEQ) and diphtheria and tetanus toxoids (+) acellular pertussis (+) inactivated poliovirus (+) hib conjugate (mpc) and rhbv vaccine (VAXELIS) experienced the temperature excursion below 10.2 degree Celsius for 0 hours 50 minutes 0 seconds. No previous temperature excursions reported.
