Symptom narrative

When training employee I observed her administer the twinrix vaccine between the deltoid and the bicep muscles. I also observed approximately 25% or less of the vaccine remaining in the syringe after the administration.

Current illness

candida rash of groin, neuropathy, uncontrolled type 2 diabetes mellitus with hyperglycemia

Medical history

Essential hypertension, uncontrolled type 2 diabetes mellitus with hyperglycemia, GERD, elevated liver enzymes, benign prostatic hyperplasia, anemia, osteoarthritis of both knees, migraine, shortness of breath, intertrigo, obstructive sleep apnea, edema of both lower legs
