Symptom narrative

Patient presents with her mother for vaccinations. Went over health history verbally prior to starting appointment. She has no allergies, medications, no health conditions, no recent vaccinations, no concerns. Did say she has anxiety about needles but does well if distracted by talking. After administering vaccine at 1214pm, I remain with the patient for observation, patient seated in room. 10 minutes after administration (1224pm), patient states she is not feeling well. Provided the patient with apple juice and fruit snacks. Patient appeared flushed, feeling hot, nauseous and lightheaded. Alerted the staff via patient communicated about patient condition to be on standby. Breathing rate increased to 24-28 breaths per minute. Worked with patient to do slow controlled breathing. Patient vomited a small amount of vomit into an emesis bag. No loss of consciousness, patient was able to communicate well through event. Patient able to slow breathing to 16-18 breaths per minute. Patient remained sitting for total of 25 additional minutes with RN and parent in the room. Patient reports feeling improved. Mother states the patient has not had a vaccine for 3 years, but she did have same reaction for all previous vaccinations in the past, always post vaccine administration.
