Symptom narrative

Patient reported vaccine reaction to this writer during a Nurse Visit appointment on 05/18/26 9:30AM for a teaching on how to use a blood pressure machine to complete at home readings Three days after administration, patient began to have redness, that spread down from injection site, to above elbow area over the 3 days. Patient describes it as painful 6/10 to touch, with warmth, itchiness, that lasted for approx. 5 days. Patient now with dry skin in the area, photos taken and added to chart. Patient using vaseline to apply to dry pealing skin area. Currently without any symptoms at this time, other than the skin pealing at the injection site.
