Symptom narrative

At approximately 3:09 pm, three vaccines (Boostrix, Gardasil, Menveo) were administered to patient. Patient complained last vaccine "really hurt" but reported he felt comfortable to stand. Patient was asked several times how he was feeling and reported "he felt good". Patient and mother proceeded to checkout with mother after waiting roughly 8 minutes in patient room. ? ? At approximately 3:18 pm, mother reported patient began leaning on her while standing at checkout and then experienced a vasovagal syncopal episode. Mother slowly assisted patient to floor. Front desk staff alerted nursing staff, who then alerted provider, Dr. ? ? Vital signs were initiated at approximately 3:20 pm while patient was laying down. BP- 118/73 mmHg HR- 88 bpm Temp- 98.3 F. Patient complained of tooth pain, Dr. examined patient's mouth. Mother reported patient did not hit head or mouth during syncopal episode. ? ? Vital signs were rechecked at approximately 3:25 pm while patient remained laying down. BP 120/74 mmHg HR 98 bpm Vital signs were checked again at approximately 3:30 pm while patient was sitting upright. BP- 110/70 mmHg HR 82 bpm Vital signs were rechecked once more at 3:35 pm while sitting in chair. BP- 117/68 mmHg HR- 81 bpm ? ? Tylenol was administered at approximately 3:40 pm per Dr.'s orders. ? Vitals rechecked at 3:40 pm. BP 108/63 mmHg HR 79 ? ? Vitals checked last time at 3:45 pm. BP 113/67 mmHg HR 99 ? ? At approximately 3:50 pm, nurse escorted patient and mother to vehicle safely.
