Patient came to clinic on 4/23/26 for his 4-month well child. Patient was due for Pediarix, Pneumonia 20, Acthib and Rotarix. Patient was given Pentacel. Pediarix, Pneumonia and Rotarix. Patients' patient did not call in regard to any adverse reactions.
