LPN reports a patient in their clinic who received the meningococcal reconstitution liquid only and never received the powder of Menveo (2 vial).; LPN reports a patient in their clinic who received the meningococcal reconstitution liquid only and never received the powder of Menveo (2 vial).; This non-serious case was reported by a nurse via call center representative and described the occurrence of inappropriate preparation of medication in a 23-month-old female patient who received Men ACWY-CRM NVS (Menveo) (batch number AMXB094A, expiry date 30-SEP-2027) for prophylaxis. On 12-MAY-2026, the patient received Menveo. On 12-MAY-2026, an unknown time after receiving Menveo, the patient experienced inappropriate preparation of medication (Verbatim: LPN reports a patient in their clinic who received the meningococcal reconstitution liquid only and never received the powder of Menveo (2 vial).) and inappropriate dose of vaccine administered (Verbatim: LPN reports a patient in their clinic who received the meningococcal reconstitution liquid only and never received the powder of Menveo (2 vial).).
