Symptom narrative

Pt reported pain and swelling at injection site and feeling fatigued after vaccine (5/15) over the weekend (5-16-518) rash had occurred and began spreading down from injection site to the sides of arms. Pt stated she alternated with heat and ice and ibuprofen with symptoms continuing to get worse. Pt returned to clinic today (5/19) and was referred to occupation health for further assessment
