Symptom narrative

Pt called the pharmacy and isn't sure if incident is connected but said she had a stroke about 24 hours after she received her first dose of shingrix vaccine and just wanted to let the pharmacy know. I told her probably not connected but would report it. She was hospitalized and is doing well. She had no permanent issues.

Medical history

chronic kidney disease stage 2, A-fib, post status stent, chronic inflammatory demyelinating polyneuropathy, osteoarthritis, essential hypertension, collagenous colitis,

Other medications

Furosemide, gabapentin, dofetilide 250, mycophenolate 360mg, rabeprazole, rosuvastatin, lisinopril 5, eliquis 5mg, prednisone 5mg

Allergies

chlorhexidine, naproxen, sulfa, morphine
