None.. please note the polio component was 5th dose. Dtap was 4th, mmr and varicella 2nd. Unsure if exact locations. Clinic may have but i was told varicella in left leg and mmr, dtap ipv in right thigh which was affected leg. Clinic also has lot numbers. Also unsure of brand names
