Two clients in clinic that received Vaxelis, one of the clients received Quadracel instead of Vaxelis. Unable to determine which client received the DTAP-IPV instead of Vaxelis. No adverse reactions were noted. 05/19/2026: This client's parent was notified of the client current vaccination status. Discussed plan for their next scheduled vaccine administration for June 2026. Answered parent questions re: vaccine client received. Parent verbalized understanding and appreciated call.
