Site: Pain at Injection Site-Mild, Additional Details: It appears that due to a communication issue, pt believed he was going to receive the polio vaccine, but instead signed up for the mmr vaccine. the exact conversation details are lost, but the pt at the time agreed to these 3 vaccines.
