Symptom narrative

Site: Bruising at Injection Site-Mild, Site: Pain at Injection Site-Medium, Additional Details: Patient states that the injection site was too low & shows bruise that is still apparent on R arm. She comments on documentation stating the vaccine was given on the L arm when it was given on the R arm. Patient states that the area was initially sore, but not at this time. Patient also states she is fearful of neuropathic damage/pain but has not experienced any tingling. Patient was advised to contact her primary care doctor for additional work up to assess any potential damage.
