Symptom narrative

Site: Pain at Injection Site-Mild, Additional Details: administered Priorix vaccine without first reconstituting with powder so patient only received diluent. Realized her mistake later after her shift and came back and reported to me. I called pt and explained to him what happened and he agreed to come back to recieve correctly reconstituted vaccine. Pt came around 530p and I administered priorix correctly to him.
