Symptom narrative

Site: Pain at Injection Site-Mild, Systemic: pt denied having any adverse effects.-Mild, Systemic: Flushed / Sweating-Mild, Additional Details: Patient was given Prevnar vaccine twice-Pts PCP asked RN to given pt the vaccine and RN later noticed that pt was already Prevnar 20 the same day. As soon as RN found out the pt was contact and informed of the error. Pt denied having any adverse symptoms at this time. RN education pt on adverse symptoms to be aware of and to call EMS if adverse reactions present. RN will can and check on pt tomorrow. The center CMD was notified d/t PM being OOO.
