Symptom narrative

Patient observed to look unwell and pale about 30 seconds after administration of HPV. Energix-B given first with Boostrix second in the left arm. Gardasil 9 given in the right arm as the last administration. Patient began to slump to the right and myoclonic movements of the upper and lower extremities noted. Patient assisted to supine position on the exam bed. Seizure lasted 15 seconds with post-ictal phase of 15 seconds. Seizure first aid provided; patient does not have prior history of seizures or adverse reactions to immunizations in the past. VS monitored and pulse noted to be bradycardic at 40 bpm radially. BP and SpO2 stable, patient remaiend afebrile with max temp of 98.4F orally. Observed over the next 50 minutes with continuous SpO2 and repeat BPs; pulse stabilized in high 60s at time of discharge. Father strongly recommended to take patient to ER for further evaluation. Patient discharged health room at 1106. As of 1350 PM, no ER visit documented.
