Symptom narrative

No additional AE; Nurse reports she inadvertently administered the GARDASIL 9 to a 15-month-old patient. The patient was supposed to receive a PREVNAR 20.; Nurse called and reported GARDASIL AE. Nurse reports she inadvertently administered the GARDASIL 9 to a 15-month-old patient. The patient was supposed to receive a PREVNAR 20. Nurse reports she already spoke to the parents of the patient and no effe; This spontaneous report was received from a nurse and refers to a 15-month-old patient of unknown gender. The patient's medical history, concurrent conditions and concomitant therapies were not provided. On 14-MAY-2026, the patient was inadvertently vaccinated with human papillomavirus 9-valent vaccine (GARDASIL 9) 0.5 ml, lot # Y015560, expiration date 16-FEB-2027 (route of administration, anatomical location were not provided) for prophylaxis. The patient was supposed to receive Pneumococcal vaccine conj 20v (PREVENAR 20) (Wrong product administered; Product administered to patient of inappropriate age). The nurse reported she already spoke to the parents of the patient and no effects were reported. She did not clarify how this medication error occurred.
