Symptom narrative

There were NO adverse effects to my knowledge. None reported by mom, nor were any observed while inside of the clinic. The patient's mother did not wait the 20 minutes after administration as advised for a shot check. Attempted to call the parent two times after the vaccine administration to check on patient, there was no answer.
