Symptom narrative

By history and physical exam it is apparent that vaccine was given too high into the soft tissue overlying the acromion and origin of the deltoid rather than into the deltoid muscle. This error appears to have occurred because of a preexisting shoulder separation. The lateral end of the clavicle was probably misidentified as the acromion so that the vaccination was given too high. This led to more pain then would normally be expected.
