Symptom narrative

The mother states the patient leaned to the left while he was sitting on the exam table while waiting 15 min for observation after vaccine administration and noted his skin was pale, his fists were clenched and his eyes were open. She assisted him and shout for help. When I went into the room he was pale, breathing normal, and not posturing. He was laid flat on the table and soon after stated he felt better, but seemed confused and did not remember the event. He was oriented x3, O2 sats were 98-100% on room air, Pulse was 56/min, and BS glucose was 79. Stated he had not had breakfast earlier and was a little nervous. Requested to drink water and drank it, but wanted to lay down again because he felt a little dizzy. 911 was contacted due to the description of seizure-like activity by the mother and the fact that he was not recovering fully, felt light-headed and did not remember the event. Emergency personnel reassessed him and was found to be stable, but the patient stated he was still feeling dizzy and preferred not to walk. The parent was given the option to take him to the hospital herself or to transport by ambulance for further evaluation. She preferred to transport the patient by ambulance.
