Symptom narrative

Patient was here for Hepatitis A, MMR and Varicella vaccine. Instead of MMR vaccine, MA administered MMRV. As a result, patient received Hepatitis A, MMRV and Varicella, duplicate Varicella. Informed mom about our mistake of duplicate doses of Varicella vaccine this morning. Mother stated patient is doing OK so far and sleeping at the moment, no sign of side effects or allergic reactions. Informed mom to monitor patient and contact us if patient develops fever or rash. ER precaution if fever higher than 102, wheezing, difficulty breathing, extreme weakness, excessive irritability, sudden staring spells, rhythmic jerking, stiffening of the body.
