Symptom narrative

The 2 vaccinations listed above were given for a second time in error. The first doses were completed on 04/27//2026. The lot numbers are the same but the sites were different. The varicella was given subcutaneously in the left vastus lateralis and the hep A was given intramuscularly in the left vastus lateralis. According to the provider that notified us of the error they are blaming it on an issue with Epic, however I can see that the same vaccine was administered twice and the dates entered. I have requested the records from the VDH and the doctors office. My daughter has had red spots at both injection sites since the day they were administered. I was only notified of the error today 05/20/2026 after the office finally left a voicemail for me to call back.
