Patient was given rotavirus vaccine one day past the expiration date. Vaccine was given on 5/18/26. The expiration date is 5/17/2026. Father notified of the error on 5/19/26.Discussed case with MERCK on 5/19/26 and 5/21/26 case number xxx and xxx respectively. We recommended to family that the child be revaccinated after 4 weeks.
