Symptom narrative

Onset of severe L5 radicular pain on 5/12/26 the day after second Shingrix immunization. MRI done not significant for lumbar structural abnormality. Shingles skin rash noted on 5/19/26 by patient, seen in clinic 5/20/26 to confirm. Patient seen 5/15/26 in clinic, ER visits on 5/17/26, 5/18/26 with follow up clinic visit 5/20/2026. Started on valtrex. Norco/gabapentin for pain control. Lidocaine patches also prescribed. Time off work for two weeks due to severe pain and functional impairment.

systemic lupus, hyperlipidemia, nephritis, depression

citalopram 20mg, lisinopril 5mg half tablet, mycophenolate 500mg two tabs in AM, 1 tab in PM, simvastatin 10mg three times a week
