Symptom narrative

Came in for office visit on 5/10/26 ~1:30pm. Received the 2 vaccines above. Went home & went for a walk. Patient starting getting fussy. Then ~4:50pm, legs then whole body started shaking. Called 911 & was transported to ER ~5pm. @ time of seizure, mom took temp & it was 101.4 degrees f.
