Symptom narrative

PT came in asking when their 2nd Shingrix dose was, they had forgotten what the appt date was. When Clinical Opportunities was pulled up to review pt it was discovered that they had already received 3 doses of shingrix. The previous 2 doses were at their previous pharmacy. The doses were about a year apart, pt had assumed that since they were not withing 6 months the vaccine would not work properly. This assumption by the patient resulted in them REQUESTING a Shingrix shot at our pharmacy. THE PT DID NOT REPORT ANY ADVERSE REACTION
