Administration of only the liquid component of Penmenvy; Administration of only the liquid component of Penmenvy; This non-serious case was reported by a other health professional via call center representative and described the occurrence of inappropriate preparation of medication in a 17-year-old male patient who received Men ABCWY NVS (Penmenvy) for prophylaxis. On 18-MAY-2026, the patient received Penmenvy. On 18-MAY-2026, an unknown time after receiving Penmenvy, the patient experienced inappropriate preparation of medication (Verbatim: Administration of only the liquid component of Penmenvy) and inappropriate dose of vaccine administered (Verbatim: Administration of only the liquid component of Penmenvy). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 19-MAY-2026 The reporter reported that a patient that was ordered the Penmenvy vaccine, but the powder and the diluent were not mixed together. So we were trying to see, if the MenB, which was the liquid part, was that still valid or do they need to get the entire vaccine because we have Menveo, with just MenACWY, we were going to give that. No further details were provided. The batch number was not provided upon follow- up with the reporter.
