On an unknown date, the patient received Arexvy. On an unknown date, an unknown time after receiving Arexvy, the patient experienced wrong solution used in drug reconstitution (Verbatim: mixed the medication with a regular sterile water diluent instead of the one that came in the box ) and inappropriate dose of vaccine administered (Verbatim: mixed the medication with a regular sterile water diluent instead of the one that came in the box). The outcome of the wrong solution used in drug reconstitution and inappropriate dose of vaccine administered were not applicable. Additional Information: GSK Receipt Date: 15-MAY-2026 The pharmacist reported that they had administered an Arexvy vaccine and noted that it came with a medication and a specific diluent. However, they believed that, by mistake, the medication had been mixed with regular sterile water instead of the diluent provided in the package. They asked whether the patient needed to be called back for another dose or if the vaccine would still be effective with the sterile water diluent that had been used. The batch number was not provided upon follow- up with the reporter.
