Symptom narrative

About 7 hours after vaccine administration she developed high fever (Tmax 103.3), rash, and fatigue. As of time of report was still experiencing these symptoms.

Current illness

Bilateral otitis media + conjunctivitis. Diagnosed on 5/11/26. Symptoms and exam improved by date of vaccine administration

Medical history

She was referred to PT for gross motor delay at her 9mo WCC and new referral made to OT for this issue on date of visit for 12moWCC/vaccines

Other medications

Augmentin 600mg/5ml suspension - 3.5ml twice daily x 10 days. Started on 5/11/26
