Symptom narrative

Patient presented with localized swelling that measured 5cmx6cm it is warm to the touch and itchy. He stated he noticed this beginning the morning after he received his shots but then it started to go away within a few days, He noticed today that it is swollen red, warm and itchy again. It has been 8 days since he was vaccinated. Patient was given topical hydrocortisone for itching. He was also instructed to do cold compresses as needed.
