Reported Symptoms: 10008531:CHILLS; 10011232:COUGHING; 10039100:RHINORRHEA; 10043898:TIREDNESS; Narrative: see above Other Relevant HX: mild COPD dx "Wednesday night coughing got worse had chills seemed like it was probably the shot and figured it would go away. no fever and the chills went away. Cough got crazy. Tired dayquil nyquil for a couple of days that probably didn't hurt anything but it did not make anything better. That covers Wed, Thur, Frid... Sat. was bad thought about ER a lot, " He thought he was getting better Mon/Tues last wk- coughing and tired- but then on Wed/Thurs was coughing more and more. Slept v little Wed because he was coughing so much, Thurs, v tired, O2 lvl in mid to high 80s wo went to ED. Went to ED Thurs- "Medical Decision Making Suspect symptoms are related to underlying COPD. Other consideration are pneumonia, CHF, pleural effusion, pneumothorax, PE anemia, among others. Patient is in no acute distress. His O2 sat at the time of my evaluation is 94% on room air. He had a chest x-ray done prior to my evaluation. I have reviewed it. No acute findings. COPD noted, but no pneumonia, effusion, pneumothorax we will check basic blood work, an EKG, and provide a DuoNeb to see if this helps with symptoms. We discussed the possibility of a course of prednisone, as I think this will help the most. Patient's labs are reviewed. No significant abnormalities. Mild hyponatremia with a sodium of 131 noted. Glucose 124, remainder of CMP unremarkable. CBC normal as well. Patient given a 500 mL bolus of normal saline. He is educated regarding results of his
