Reported Symptoms: 10006791:BURNING SENSATION SKIN; 10022053:INJECTION SITE BURNING; 10037844:RASH; Narrative: middle-age male patient past medical history GERD chronic recurrent low back pain contact dermatitis vitamin D deficiency BPH additional medical problems noted below followed in facility primary care clinic Presents urgent care center with a 3-day history of 2 specific issues #1 Developed painful discomfort right upper arm deltoid region without localized injury He had received a immunization for RSV 1 week ago but the injection was placed in his contralateral arm A pneumococcal vaccine was administered in the right arm however this was nearly 4 weeks ago and he had only developed localized swelling for past 3 to 4 days #2 Developed a moderately pruritic eruption truncal area which is also causing a mildly burning sensation Denies recent topical treatments Of note patient finished a 2-week course of metronidazole/bismuth subsalicylate/tetracycline approximately 10 days ago in treatment of H. pylori; treatment was initiated on February 12 and he developed the symptoms of rash approximately 4 weeks after initiating treatment with these meds On focused ROS no fever chills weakness shortness of breath arthralgias noted On review of chart no true allergies to medications appreciated however did note symptoms of dizziness with use of tamsulosin and had stopped it Is noted to have a history of contact dermatitis related to suspected poison ivy exposure in 2011; today he notes no topical exposures to medications or fabrics Other Relevant HX:
