Symptom narrative

Reported Symptoms: 10012727:DIARRHEA; 10047700:VOMITING; 10062369:EPOPHAGEAL ACID REFLUX; Narrative: Patient rec'd vaccines (pneumococcal and RSV) on Tuesday and reports being sick since with vomiting and diarrhea. He was unable to eat much and after he ate everything is burped up like acid reflux. Denies any fever, body aches or chills, but reported no energy. Lost 15 lbs over the week d/t illness. Other Relevant HX: He had recurrence of lymphoma after noticing a lump on his neck in June 2025. He has been treated at (name withheld). Completed chemo and radiation in December.
