Symptom narrative

Patient was given Pentacel in right thigh without incident. While giving Prevnar, patient twisted left thigh while MA was pushing plunger to administer the medication; this caused the needle to leave the thigh and about half of the dose was lost. No patient injury, no medical symptoms. No adverse reactions to the small dose that patient recieved.
