Symptom narrative

The incorrect vaccine was administered in error by medical assistant. The practice manager called Sanofi, the manufacturer of Vaxelis, to investigate any possible contraindications, side effects or any information that would need to be provided to the patient's parents. Place call to (number withheld), followed the prompts to speak to representative regarding vaccine concern, spoke with agent, provided information regarding immunizations orders vs. immunizations administered and non-identifying patient information (ie- 15 month old female patient). Agent verbally advised that aside from slight increase in the normal side effects (i.e- low grade fever, site tenderness, site redness/warmth), there were no additional concerns, contraindications or side effects. APRN called the mother of patient to notify of vaccine error and documented within the patient's visit note for the date of service of May 7, 2026. APRN expressed to this practice manager that the mother of the patient, while seemingly shocked, didn't have questions at the time.
