Symptom narrative

Patient called on 5/22/26- With inquiry about side effects from a covid vaccine, that may have resulted in an injury to her right arm. Patient states she has had a slow onset of pain and weakness to the entire right arm. Patient was unable to identify the actual onset but stated it has gradually been getting worse, which is resulting in weakness at times, pain is positional and is noticeable from the right should down to the wrist at times. Patient denies any color changes to the right arm, no numbness and has no other neurological symptoms. Patient has mentioned this to her primary APN, with no observed acute injury from a fall or trauma. Patient does not recall any redness, no rash and no observed concerns after the initial injection. States she had the usual soreness after any routine vaccine. Previous vaccine through her primary APN was October and that was the influenza vaccine. Patient is taking Eliquis for Atrial Fibrillation and states her blood levels have been good and in range.
