Symptom narrative

she describes site at about 3 to 4 inch area and " like a big old splot, it is a bit swollen, it has gone down since yesterday". she reports she noticed the rash last night at about 11pm after getting home from her work shift. she states she work a jacket during her shift so she did not notice until she returned home. she describes reports the site is not painful, nor itchy,. she denies presence of hives, fever, shortness of breath, tongue swelling, difficulty breathing. she reports compared to last night, "it seems to be clearing away". As noted above, she notes swelling has reduced. Per she she has take n ibuprofen and Tylenol last night. she has not taken any Bendaryl thus far. she also reports that she received the Adacel TDAP vaccine at her PCPs office shortly before receiving vaccines at Employee Health in April 2026. Date on TDAP record provided by she at pre-employment appt was 4/8/2026. Vaccines were admisntered to she at pre-employment appt 4/21/2026. At that time, she there was no reported or observed reaction. Prior to administering second second dose on 5/21/2026 , OHN completed consents with she and also inquired if she had any reaction or issue with the first dose. she denied. she reports she now recalls that after the first vaccine a month ago, she experienced the same swelling and redness on her left arm however it only lasted one day and went away.
