Symptom narrative

Mom called today to report that patient started having redness and swelling to the injection site in her left thigh on the date of immunization 2 days ago. It has since become more swollen and red. She denies any other symptoms at this time. I called this in to the State where ice was recommended as well as monitoring the site. It was recommended that they go to the ER if she has any other symptoms.

Current illness

Mom says she has had several colds and illnesses recently. Maybe up to 4 or 5 times back to back. Mom says first time she got sick was in Feb while they were in (withheld country). She was throwing up.
