Symptom narrative

Patient had his pediarix vaccine on 5/19/26 in the right deltoid and by the end of the day he started c/o fatigue, malaise, headache, fever and started developing right shoulder pain and redness of the right deltoid where the vaccine was given. He was seen in the office on 5/22/26 as symptoms were not improving. The patient was examined and the reaction was documented. He had redness and swelling overlying the right lateral deltoid on exam. He was afebrile in the office but mom just gave his tylenol prior to the visit. He appeared mildly fatigued. rom in shoulder was limited due to pain.
