Symptom narrative

mild redness at site, no swelling, some itching. patient came into pharmacy to show me reaction, no other redness on body according to pt, patient stated redness had gone down since shot was given. I advised patient to go see dr if reaction got any worse, redder, hot to touch or red outside the borders it was at the time. Patient stated redness ha stayed within same borders the whole time and was flat, no swelling just rash like. I suggested ice and a benadryl if it remained itchy.
