Symptom narrative

No other AE; administered MMRV (PROQUAD) instead of MMR-II; confusion with the product names; gave a proquad to a 13 year old by mistake; This Spontaneous report was received from Nurse Practitioner and referred to a 13-year-old Male (also reported as "she") patient. The patient's medical history was not reported. The patient's past medications were not reported. The patient's concomitant medications were not reported. On 15-May-2026, the patient was vaccinated with MMR and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) (0.50 mL) (total) (Batch/Lot Number: Z011678) (Expiration date: 10-Jan-2027) with sterile diluent solution for injection by mistake for prophylaxis (Product administered to patient of inappropriate age). This patient was due to his second dose of measles, mumps, and rubella (wistar ra 27-3) virus vaccine (MMR-II), however, the nurse confused with the product names (Product name confusion) and administered MMR and Varicella (Oka-Merck) Virus Vaccine Live (PROQUAD) to the patient (Wrong product administered). No adverse event was reported.
