Symptom narrative

Right thigh: Large, well-circumscribed area of erythema centered at the DTaP injection site, approximately softball-sized (estimated 10?12 cm in diameter). Erythema is diffuse with a slightly deeper pink-red hue centrally, fading to lighter pink at the periphery. Borders are round and relatively well-defined. Approx 3cm area of induration centrally. Mildly warm, nontender. No vesiculation, bullae, or skin breakdown noted. No red streaking. No fluctuance. Surrounding skin appears normal.
