mistakenly administered only the reconstitution agent; mistakenly administered only the reconstitution agent; This non-serious case was reported by a nurse via other manufacturer and described the occurrence of inappropriate preparation of medication in a patient who received Herpes zoster (Shingrix) for prophylaxis. On 20-MAY-2026, the patient received Shingrix. On 20-MAY-2026, an unknown time after receiving Shingrix, the patient experienced inappropriate preparation of medication (Verbatim: mistakenly administered only the reconstitution agent) and inappropriate dose of vaccine administered (Verbatim: mistakenly administered only the reconstitution agent). The outcome of the inappropriate preparation of medication and inappropriate dose of vaccine administered were not applicable.
