Symptom narrative

PT fainted right after vaccination was finished. PT was seated and was caught by the TECH. TECH yelled for the RPH. RPH came and the PT woke up shortly after. PT was unconscious for more than 30 secs. but less than 60secs. PT was breathing entire time she was unconscious (PT was snoring.) RPH and TECH observed PT for 15mins and gave her water. PT said this has happen before and that she felt fine and left afterwards. The incident was also documented with the nurse triage line.
